Notify SLC Arts as soon as possible if any changes need to be made to your program.
Are you an artist getting paid by a grant? You need to sign this form so we can legally pay you.
Survey your audience (not required, but encouraged).
Expect a visitor from SLC Arts to come see your program. Here’s what they’ll be looking for:
